    How can I find out who my student's case manager is and how to contact them?
    In the fall, a letter will be sent home with the name and contact information for your student's case manager.  If you can't find that letter, please contact the special education department leader, Lynn Florman, at Lynn.Florman@ahschools.us or 763-506-6692 and give her the name of your student.  She will call you and give you the information, or reply to your email and attach your case manager to the email so that you have their name and contact information.
     
    How can I get a copy of my student's Individualized Education Plan (IEP) and Evaluation Summary Report (ESR)?
    Your student will have a new IEP every year, and a new ESR every 3 years.  To get a copy of your student's most recent IEP and ESR, please email Lynn Florman at Lynn.Florman@ahschools.us or call her at 763-506-6692 and give her your student's name and grade.  Your student's IEP and ESR can be emailed to the email address you provided to the school, or copies of these documents can be sent home with your student in a sealed envelope or sent directly to you via US Mail.
     
    How do I get accommodations for high stakes testing (ACT, SAT or AP tests)?

    How do I know if my student's teacher has seen his or her IEP?
     All teachers have access to a summary of each student's IEP and Health Plan through Viewpoint, and are required to read them before the trimester begins.  Instructions about how to access your student's IEP or Health Plan through Viewpoint are sent to all teachers at the beginning of the school year.  Most teachers have a chart or spreadsheet that they use to keep track of which accommodations and modifications are outlined in each student's IEP or Health Plan so that they can refer to it.  If you are unsure if your student's teacher has seen your student's IEP or Health Plan, you can ask them about it in person during the open house or conferences.  However, you are always welcome to email your students teachers to ask if they have any questions regarding your student's accommodations or modifications.  
     
    How can I get a schedule change for my student?
    Counselors have the ability to make schedule changes for your student, but will contact your student's case manager to ensure that the schedule change is coordinated with your student's IEP.  Your student's case manager does not have the ability to make a schedule change, but they can request one for your student through your student's counselor or the special education department lead after reviewing your student's IEP.
     
    How can I make a change to my student's IEP?
    Your student's IEP will be reviewed annually, but a significant change can be made to your student's IEP at any time during the year if their goals, services, accommodations or modifications change before their annual IEP meeting.  Please contact your student's case manager if you would like to request a change to their IEP.
     
    How do I find my student's grades and progress toward graduation?
    You can access your student's grades and progress toward graduation by going to the district website at www.ahschools.us and clicking on the Lock and Key in the upper right corner of the page.  A new window will pop up and you will be prompted to enter the login information you received from the district at the beginning of the school year.  If you don't remember your login information, you can contact the technology help desk at 763-506-HELP (4357) during regular business hours and they will help you create a new login.  You can also download the Anoka-Hennepin Schools App and access your student's grades as well, although you will need to know your login information to access the information on the app as well.
