Traditional Private Schools
Traditional private schools located within the Anoka-Hennepin School District boundaries are eligible to receive state supports for health, guidance, and textbook aid. Aid may be requested by observing the following forms and deadlines.
- Request Form for Textbooks, Standardized Tests, and Individual Instructional Materials; Request Form for District Pupil Health Services; Request Form for District Secondary Guidance/Counseling Services. All three forms are to be completed by parents and returned to the nonpublic school by September 15 each year. Data obtained from these forms will be used to complete the Student Report for Aids to Nonpublic Students.
- Student Report for Aids to Nonpublic Students (MDE form ED-01650). Using parent supplied information found in the previously mentioned Request Forms, this aid request form must be completed by the nonpublic school and returned to the Anoka-Hennepin Home and Nonpublic School Office by October 1 each year.
